主动脉全弓置换联合冠状动脉旁路移植术的早期治疗效果

【摘要】目的  探讨主动脉全弓置换手术(TAR)联合冠状动脉旁路移植术(CABG)的早期治疗效果。方法  回顾性分析2009年1月至2011年12月于首都医科大学附属北京安贞医 院择期行TAR联合CABG的50例患者的病历资料。分析患者手术资料和TAR联合CABG的临床效果。结果  所有患者手术时间(230±78)min,体外循环时间(153±52)min,冠状动 脉远端吻合(2.1±1.1)次。患者手术死亡3例,分别于术后1 d死于心室颤动、术后3 d死于围术期心肌梗死、术后16 d死于多器官功能衰竭,主要并发症10例。术后早期移植 物通畅率为100%。结论  TAR联合CABG可以安全地进行并有良好的术后早期临床效果。

Clinical early outcomes of total aortic arch replacement with coronary artery bypass grafting

【Abstract】Objective    To analyze clinical characteristics and efficacy of total aortic arch replacement(TAR) combined with coronary artery bypass  grafting(CABG). Methods    Totally 50 patients who had TAR and CABG from January 2009 to December 2011 in Beijing Anzhen Hospital, Capital Medical  University were retrospectively enrolled. Operation data and postoperative efficacy were analyzed. Results    Operation duration was (230±78)min;  cardiopulmonary bypass duration was (153±52)min; the occurrence of coronary artery distal anastomosis was (2.1±1.1)times. There were 3 death cases,  including 1 case of ventricular fibrillation 1 d after operation, 1 case of perioperative myocardial infarction 3 d after operation and 1 case of multiple  organ failure 16 d after operation. There were 10 cases of major complications. Postoperative early graft patency rate was 100%. Conclusions    TAR  combined with CABG is safe and effective.
