蒿芩清胆汤联合克林霉素磷酸酯治疗盆腔炎性疾病后遗症的效果及对血清辅助性T细胞1/辅助性T细胞2和粒细胞-巨噬细胞集落刺激因子水平的影响

• 【摘要】目的    探究蒿芩清胆汤联合克林霉素磷酸酯治疗盆腔炎性疾病后遗症的效果及对血清辅助性T细胞1/辅助性T细胞2（Th1/Th2）和粒细胞-巨噬细胞集落刺激因子（GM-CSF）水平的影响。方法    选取北京市和平里医院2016年5月至2017年5月收治的盆腔炎性疾病后遗症患者100例，按照随机数字表法分为观察组和对照组，每组50例。对照组患者口服克林霉素磷酸酯片治疗2周；观察组患者在对照组基础上给予蒿芩清胆汤进行治疗，20 d为1个疗程，治疗1个疗程。所有患者随访3个月，观察2组患者治疗前后的症状及体征积分、血清Th1/Th2以及GM-CSF水平。结果    观察组治疗后1、2、3个月症状积分、体征积分明显低于对照组［治疗后1个月：（15±4）分比（16±4）分、（12.2±3.2）分比（13.0±3.2）分，治疗后2个月：（12±3）分比（15±4）分、（10.4±2.6）分比（12.4±2.9）分，治疗后3个月：（9±3）分比（12±3）分、（9.8±2.4）分比（11.2±2.5）分］，差异均有统计学意义（均P＜0.05）。治疗后2、3个月时，观察组Th1/Th2明显高于对照组［（0.12±0.08）比（0.10±0.08）、（0.12±0.09）比（0.11±0.08）］，差异均有统计学意义（均P＜0.05）。治疗后1、2、3个月时，观察组GM-CSF水平明显低于对照组［（24.2±3.2）ng/L比（27.4±3.0）ng/L、（21.3±2.9）ng/L比（26.4±2.9）ng/L、（19.2±2.6）ng/L比（24.2±3.0）ng/L］，差异均有统计学意义（均P＜0.05）。结论    蒿芩清胆汤联合克林霉素磷酸酯能有效提高盆腔炎性疾病后遗症患者的治疗效果，降低血清Th1/Th2及GM-CSF水平。

• 【Abstract】Objective    To investigate the effect of Haoqin Qingdan decoction combined with clindamycin phosphate on helper T cell 1/helper T cell 2（Th1/Th2） and granulocyte-macrophage colony stimulating factor（GM-CSF） in patients with chronic pelvic inflammatory disease. Methods    A total of 100 patients with sequelae of pelvic inflammatory disease admitted in Beijing Hepingli Hospital from May 2016 to May 2017 were randomly divided into observation group and control group, with 50 cases in each group. The control group took clindamycin phosphate tablets for 2 weeks; the observation group was treated with clindamycin phosphate tablets plus Haoqin Qingdan decoction, 1 course of herbal medicine was 20 d. All patients were followed up for 3 months; scores of symptoms and signs, serum levels of Th1/Th2 and GM-CSF were analyzed. Results    Scores of symptoms and signs 1, 2, 3 months after treatment in observation group were significantly lower than those in control group［1 month after treatment：（15±4） vs （16±4）, （12.2±3.2） vs （13.0±3.2）; 2 months after treatment：（12±3） vs （15±4）, （10.4±2.6） vs （12.4±2.9）; 3 months after treatment：（9±3） vs （12±3）, （9.8±2.4） vs （11.2±2.5）］（P＜0.05）. Th1/Th2 levels 2, 3 months after treatment in observation group were significantly higher than those in control group［（0.12±0.08） vs （0.10±0.08）, （0.12±0.09） vs （0.11±0.08）］（P＜0.05）. GM-CSF levels 1, 2, 3 months after treatment in observation group were significantly lower than those in control group［（24.2±3.2）ng/L vs （27.4±3.0）ng/L, （21.3±2.9）ng/L vs （26.4±2.9）ng/L,（19.2±2.6）ng/L vs （24.2±3.0）ng/L］（P＜0.05）. Conclusion    Haoqin Qingdan decoction combined with clindamycin phosphate treating patients with sequelae of pelvic inflammatory disease can effectively improve the therapeutic effect and lower serum levels of Th1/Th2 and GM-CSF.
