坤泰胶囊联合尼尔雌醇治疗对更年期综合征患者性激素和血脂水平的影响

• 【摘要】目的    探讨坤泰胶囊联合尼尔雌醇治疗对更年期综合征患者性激素和血脂水平的影响。方法    选择2016年4月至2017年7月鄂东医疗集团市妇幼保健院妇女保健科收治的更年期综合征患者86例。按照随机数字表法将患者分为对照组和观察组，各43例。在基础治疗基础上，对照组患者给予口服尼尔雌醇片进行治疗，而观察组患者给予坤泰胶囊联合尼尔雌醇片口服治疗。统计2组患者的临床总有效率，同时比较2组患者治疗前后的性激素［雌二醇、卵泡刺激素（FSH）及促黄体生成素（LH）］、血脂［总胆固醇、三酰甘油、低密度脂蛋白胆固醇（LDL-C）和高密度脂蛋白胆固醇（HDL-C）］及肝肾功能指标［天冬氨酸转氨酶（AST）、丙氨酸转氨酶（ALT）、血尿素氮和血肌酐］水平，另外观察2组患者治疗期间的不良反应发生情况。结果    观察组临床总有效率高于对照组［88.4%（38/43）比69.8%（30/43）］，差异有统计学意义（P<0.05）。与治疗前比较，治疗后2组患者血清雌二醇和HDL-C水平明显升高，而血清FSH、LH、总胆固醇、三酰甘油和LDL-C水平明显降低，同时观察组患者治疗后性激素和血脂水平改善情况明显优于对照组［（70±11）ng/L比（65±12）ng/L、（1.8±0.5）mmol/L比（1.4±0.5）mmol/L、（40±14）IU/L比（48±15）IU/L、（17±5）IU/L比（19±6）IU/L、（4.7±0.9）mmol/L比（5.3±1.1）mmol/L、（1.3±0.5）mmol/L比（1.8±0.6）mmol/L、（3.0±0.6）mmol/L比（3.4±0.6）mmol/L］，差异均有统计学意义（P<0.05）；2组患者治疗前后AST、ALT、血尿素氮及血肌酐水平差异均无统计学意义（均P>0.05）；2组患者不良反应发生率差异无统计学意义（P>0.05）。结论    坤泰胶囊联合尼尔雌醇可通过有效调节性激素和血脂水平，从而改善更年期综合征患者临床症状，用药安全性良好。

• 【Abstract】Objective    To explore the effect of Kuntai capsule combined with nilestriol on sex hormone and blood lipids in patients with climacteric syndrome. Methods    From April 2016 to July 2017, 86 climacteric syndrome patients admitted in Maternal and Child Health Care Hospital of Edong Healthcare Group were randomly divided into control group and observation group, with 43 cases in each group. On the basis of routine treatment, the control group took nilestriol; the observation group took Kuntai capsule and nilestriol. Clinical efficacy, sex hormones［estradiol（E2）, follicular stimulating hormone(FSH), luteinizing hormone(LH)］, blood lipids［total cholesterol(TC), triglyceride(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C)］, liver and kidney function［aspartate aminotransferase(AST), alanine aminotransferase(ALT), blood urea nitrogen(BUN), serum creatinine(Scr)］ and adverse reactions were analyzed. Results    The total effective rate in the observation group was significantly higher than that in the control group［88.4%（38/43） vs 69.8%（30/43）］（P<0.05）. After treatment, levels of E2 and HDL-C significantly increased while FSH, LH, TC, TG and LDL-C decreased in both groups; the improvements of above indexes in the observation group were more significant than those in the control group［（70±11）ng/L vs （65±12）ng/L, （1.8±0.5）mmol/L vs （1.4±0.5）mmol/L, （40±14）IU/L vs （48±15）IU/L, （17±5）IU/L vs （19±6）IU/L, （4.7±0.9）mmol/L vs （5.3±1.1）mmol/L, （1.3±0.5）mmol/L vs （1.8±0.6）mmol/L, （3.0±0.6）mmol/L vs （3.4±0.6）mmol/L］(all P<0.05). There were no significant differences of AST, ALT, BUN and Scr(P>0.05). There was no significant difference of adverse reaction rate between the two groups(P>0.05). Conclusion    Kuntai capsule combined with nilestriol can effectively improve clinical symptoms of climacteric syndrome through regulating sex hormones and blood lipids with a good safety.
