联合检测超氧化物歧化酶和同型半胱氨酸及高敏C反应蛋白在冠状动脉粥样硬化性心脏病诊断中的价值

  • 【摘要】目的    探讨血清超氧化物歧化酶(SOD)、同型半胱氨酸(Hcy)、高敏C反应蛋白(hs-CRP)联合检测在冠状动脉粥样硬化性心脏病(冠心病)诊断中的应用价值。方法    选取2017年2月至2018年3月山东省肥城市人民医院心内科收治的105例冠心病住院患者(观察组)和90例健康体检者(对照组),测定其血清中SOD、Hcy、hs-CRP水平,进行统计分析。结果    观察组血清Hcy和hs-CRP水平明显高于、SOD水平明显低于对照组[(21.2±4.1)kU/L比(8.4±1.8)kU/L、(21.5±4.5)μmol/L比(2.2±1.5)μmol/L、(97±12)mg/L比(110±11)mg/L],差异均有统计学意义(均P<0.01)。受试者工作特征曲线分析显示,SOD、Hcy、hs-CRP诊断冠心病的曲线下面积(95%置信区间)分别为0.631(0.553~0.709)、0.705(0.631~0.780)、0.809(0.751~0.867),敏感度分别为51.4%、59.0%、56.2%,特异度分别为82.4%、91.6%、92.6%,约登指数分别为33.8、50.6、48.8,三者联合检测的敏感度最高(74.8%)、特异度最强(96.2%)、曲线下面积最大(0.901)、约登指数最大(71.0)。结论    联合检测SOD、Hcy、hs-CRP三项指标可以进一步提高冠心病诊断的准确性。

  • 【Abstract】Objective    To investigate the value of combined detection of serum superoxide dismutase(SOD), homocysteine(Hcy) and high sensitive C-reactive protein(hs-CRP) in the diagnosis of coronary atherosclerotic heart disease(CHD). Methods    From February 2017 to March 2018, 105 CHD patients and 90 healthy people were enrolled in Feicheng People′s Hospital. Levels of serum SOD, Hcy and hs-CRP were tested and their diagnostic values were analyzed. Results    Levels of serum Hcy and hs-CRP in CHD patients were significantly higher and SOD was significantly lower than those in healthy people[(21.2±4.1)kU/L vs (8.4±1.8)kU/L, (21.5±4.5)μmol/L vs (2.2±1.5)μmol/L, (97±12)mg/L vs (110±11)mg/L](P<0.01). Receiver operating characteristic curve showed that the areas under curve(95% confidence interval) of SOD, Hcy and hs-CRP in diagnosing coronary heart disease were 0.631 (0.553-0.709), 0.705 (0.631-0.780), 0.809 (0.751-0.867); the sensitivities were 51.4%, 59.0%, 56.2%; the specificities were 82.4%, 91.6%, 92.6%; Yoden indexes were 33.8, 50.6, 48.8, respectively; combined detection of the 3 indicators showed the highest sensitivity(74.8%), specificity(96.2%), area under the curve (0.901) and Youden index(71.0). Conclusion    Combined detection of SOD, Hcy and hs-CRP can improve the diagnostic accuracy for CHD.
