中药预防和治疗获得性免疫缺陷综合征口腔黏膜病损的系统评价

• 摘要：

• 目的探究中药预防与治疗对获得性免疫缺陷综合征（AIDS）口腔黏膜病损的临床疗效。方法检索中英文数据库［中国知网、万方数据库、维普中文科技期刊数据库、中国生物医学文献数据库， PubMed, Cochrane Central Register of Controlled Trials, Embase (via Ovid), Science Citation Index Expanded (Web of Science)］中有关中药治疗或预防AIDS口腔黏膜病损的相关文献，使用Cochrane risk of bias工具评价研究的方法学质量，使用RevMan 5.3软件对符合纳入标准的文章进行Meta分析。结果 共纳入6篇文献，涉及5项随机对照试验，710例患者。Meta分析结果显示，中药组治疗后念珠菌镜检阳性率明显低于西医组（以治疗为目的，风险比＝0.45，95%置信区间0.23～0.86，P=0.02；以预防为目的，风险比＝0.10，95%置信区间0.01～0.84，P=0.03）。中药组的口腔病损有效率明显高于西医组（以治疗为目的，风险比=1.79，95%置信区间1.11～2.87，P=0.02）。结论中药在缓解和预防AIDS口腔病损方面，较西医治疗可能更有优势。由于纳入文献质量不高，所获得结论的可靠性低。

• ObjectiveTo explore the effect of Chinese herbal medicine on oral mucositis in patients with acquired immune deficiency syndrome(AIDS). Methods Literatures of Chinese herbal medicine treating or preventing oral mucositis of AIDS were searched in domestic and foreign databases［CNKI(China National Knowledge Infrastructure), Wanfang Data, VIP Chinese Sci-tech Periodical Database, China Biology Medicine disc, PubMed, Cochrane Central Register of Controlled Trials, Embase (via Ovid), Science Citation Index Expanded (Web of Science)］. Methodological quality of randomized controlled trials（RCTs） was assessed by Cochrane risk of bias. Meta analysis was performed using RevMan 5.3 sofeware. Results  Six literatures involving 5 RCTs and 710 patients were included. Meta analysis revealed that the positive rate of Candida detected by microscopy after treatment with Chinese herbal medicine was significantly lower than with western medicine（risk ratio＝0.45, 95% confidence interval 0.23-0.86, P=0.02 for treatment； risk ratio＝0.10, 95% confidence interval 0.01-0.84, P=0.03 for prevention）. The therapeutic effective rate on oral mucosal lesions with Chinese herbal medicine was significantly higher than with western medicine（risk ratio=1.79, 95% confidence interval 1.11-2.87, P=0.02）. Conclusions  In terms of relieving and preventing oral mucositis, Chinese herbal medicine shows advantages over western medicine treatment. The effect of Chinese herbal medicine on AIDS patients with oral mucositis remains unknown.
