肺静脉血栓致急性肠系膜上动脉栓塞1例报告

• 摘要：

• 肺静脉血栓形成(PVT)是一种罕见但具有潜在致命性的疾病，最常作为恶性肿瘤、肺手术后或心房颤动的并发症发生。在此，我们报道1例罕见PVT病例，该患者是1例患有肺部恶性肿瘤的中年男性，行左全肺切除术后患者出现腹痛、腹胀的症状，并诊断为急性肠系膜上动脉栓塞。根据这一病例报告，我们认为在肺部恶性肿瘤手术切除过程中应在搬动肺组织之前先离断肺静脉、阻断肺动脉，并尽可能靠近左心房，能有效阻止栓子脱落至左心房，减少急性肠系膜上动脉等外周组织器官栓塞导致严重并发症的可能。本例报道以期为肺癌合并肺静脉血栓的治疗提供临床参考，从而改善患者预后。

• Pulmonary venous thrombosis (PVT) is a rare but potentially fatal disease; it most often occurs as a complication of malignant tumor, lung surgery or atrial fibrillation. Here, we report a rare PVT case. This patient is a middle-aged man with pulmonary malignant tumor; after left pneumonectomy, the patient developed symptoms of abdominal pain and abdominal distension, and was diagnosed as acute superior mesenteric artery embolism. According to this case report, we believe that in the process of surgical resection of lung malignant tumors, we should disconnect the pulmonary vein and block the pulmonary artery before moving the lung tissue, and be as close to the left atrium as possible, which can effectively prevent the thrombus from falling off to the left atrium and reduce the possibility of serious complications caused by acute superior mesenteric artery and other peripheral tissue and organ embolism. This case report is expected to provide clinical reference for the treatment of lung cancer complicated with PVT, so as to improve the prognosis of patients.
