白血病患者化疗后合并化脓性髋关节炎2例及文献复习

• 摘要：

• 粒细胞缺乏感染是血液恶性肿瘤患者化疗后的一种严重并发症，其进展迅速，可能导致危及生命的并发症。其中少见部位的感染，由于诊断困难，经常出现误诊、漏诊等情况。目前正常人群中化脓性髋关节炎案例较少，而白血病化疗后合并化脓性髋关节炎更为罕见，相关报道极少。本研究介绍了2例因白血病入院化疗后出现发热伴关节疼痛患者的诊疗经过，持续关注其体温及感染指标的变化、抗感染方案的改变，最终通过病理证实为化脓性髋关节炎。同时通过对相关文献进行复习总结，以期提高临床对粒细胞缺乏期罕见部位感染的识别和诊疗的认识。

• Agranulocytosis infection is a serious complication after chemotherapy in patients with hematological malignancies, which progresses rapidly and may lead to life-threatening complications. Due to the difficulty in diagnosis, the infection in rare sites is often misdiagnosed and missed diagnosed. At present, there are few cases of pyogenic hip arthritis in normal population, and suppurative hip arthritis after chemotherapy of leukemia is even rarer, and there are few relevant reports. This study introduces the 2 cases admitted to hospital because of leukemia after chemotherapy fever with joint pain in patients with diagnosis and treatment, continuous attention to the body temperature and infection index changes, the change of plan to fight infection, ultimately through pathology confirmed for suppurative hip arthritis. At the same time, the relevant literature was reviewed in order to improve the clinical recognition, diagnosis and treatment of rare site infection in the agranulocytosis stage.
