高脂血症是代谢性疾病中常见的一种类型,根据其临床特征可将其归属于“膏脂”“浊脂”范畴,其核心病机“痰瘀互结”深刻揭示了疾病从浊脂内蕴、痰浊滋生、瘀血内停,最终痰瘀胶结的动态演变过程。随着现代分子生物学、系统生物学等学科的飞速发展,从生物学层面阐释“痰瘀互结”的科学内涵已成为中西医结合研究的前沿与热点。本综述系统梳理了“痰瘀互结”病机与现代医学中脂质代谢紊乱、炎症反应、肠道菌群失调、内皮功能障碍及血流动力学变化等领域的关联性研究进展。同时剖析了该研究领域中存在的争论,包括“痰”与“瘀”的生物学界定模糊性、动物模型与临床实际的脱节、微观指标与宏观证候的整合困境,以及中药复方多靶点作用机制解析的挑战,以期为后续临床研究奠定一定的理论基础。
Hyperlipidemia is a common type of metabolic disease, which can be categorized into the scope of "greasy lipid" and "turbid lipid" according to its clinical characteristics. Its core pathogenesis of "intermingled phlegm and blood stasis" profoundly reveals the dynamic evolution process of the disease from internal accumulation of turbid lipid, generation of phlegm turbidity, internal retention of blood stasis to the final intermingling of phlegm and blood stasis. With the rapid development of modern molecular biology, systems biology and other disciplines, explaining the scientific connotation of "intermingled phlegm and blood stasis" from the biological perspective has become a frontier and hotspot in integrated traditional Chinese and western medicine research. This review systematically summarizes the research progress on the correlation between the pathogenesis of "intermingled phlegm and blood stasis" and fields such as lipid metabolism disorder, inflammatory response, intestinal flora imbalance and hemodynamic changes in modern medicine. Meanwhile, it analyzes the controversies existing in this research field, including the ambiguous biological definition of "phlegm" and "stasis", the disconnection between animal models and clinical practice, the dilemma of integrating micro indicators with macro syndromes, and the challenges in analyzing the multi-target mechanism of action of traditional Chinese medicine compound prescriptions, aiming to lay a certain theoretical foundation for subsequent clinical research.
