  • 糖尿病溃疡是糖尿病常见的并发症之一,具有高死亡率、高截肢率和高复发率的特点。内质网应激(ERS)是机体维持细胞稳态的一种生理调节,而炎症是机体清除坏死物质的自我免疫应答。生理状态下,一定程度的ERS和炎症有利于细胞的新生。在糖尿病溃疡发病进程中,高血糖环境和过度的炎症引起的ERS不断增强,造成一系列下游病理反应是糖尿病溃疡不愈合的关键。近年研究发现,ERS与炎症的相互串扰与糖尿病溃疡的愈合密切关联。ERS通过影响巨噬细胞极化、核因子κB通路和NOD样受体蛋白3炎性小体参与糖尿病溃疡的愈合。文献表明,ERS抑制剂、中药单体、中药复方以及新兴技术疗法通过调控ERS-炎症的交互作用,改善糖尿病溃疡的愈合状况。基于此,本文对ERS与炎症交互作用在糖尿病溃疡中的分子机制和药物靶点的研究进展进行综述,以期为糖尿病溃疡的治疗提供新的思路。

  • Diabetic ulcer is one of the common complications of diabetes, characterized by high mortality, high amputation rate and high recurrence rate. Endoplasmic reticulum stress (ERS) is a physiological regulation for the body to maintain cellular homeostasis, while inflammation is a self-immune response of the body to clear necrotic substances. Under physiological conditions, a certain degree of ERS and inflammation are beneficial to cell neogenesis. During the pathogenesis of diabetic ulcer, the high glucose environment and excessive inflammation lead to the continuous enhancement of ERS, and the resulting series of downstream pathological reactions are the key factors for the non-healing of diabetic ulcer. In recent years, studies have found that the crosstalk between ERS and inflammation is closely related to the healing of diabetic ulcer. ERS participates in the healing of diabetic ulcer by affecting macrophage polarization, nuclear factor κB pathway and NOD-like receptor protein 3 inflammasome. Literature shows that ERS inhibitors, Chinese medicine monomers, Chinese medicine compound prescriptions and emerging technical therapies can improve the healing of diabetic ulcer by regulating the interaction between ERS and inflammation. Based on this, this article reviews the research progress on the molecular mechanism and drug targets of the interaction between ERS and inflammation in diabetic ulcer, in order to provide new ideas for the treatment of diabetic ulcer.
