目的 探讨老年急性心肌梗死(AMI)患者经皮冠状动脉介入(PCI)术前后外周血具有11个配体结合重复结构的低密度脂蛋白受体相关蛋白(LR11)、蛋白酪氨酸磷酸酶受体O(PTPRO)水平变化及其与支架内再狭窄(ISR)的关系。方法 前瞻性选取2021年1月至2023年12月在西安交通大学附属红会医院行PCI术的老年AMI患者312例,根据1年后是否发生ISR分为ISR组和非ISR组。采用酶联免疫吸附试验法检测外周血LR11、PTPRO水平。通过Spearman相关分析AMI患者外周血LR11、PTPRO水平与PCI术后1年管腔直径狭窄率的相关性,并分析外周血LR11、PTPRO水平与AMI患者PCI术后ISR的关系及预测效能。结果 AMI患者PCI术后外周血LR11、PTPRO水平高于术前[(195±48)mg/L比(153±43)mg/L、(20±4)μg/L比(16±4)μg/L](均P<0.05)。AMI患者PCI术后外周血LR11、PTPRO水平与PCI术后1年管腔直径狭窄率均呈正相关(rs=0.617、0.602,均P<0.001)。随访1年,312例AMI患者PCI术后ISR发生率为16.7%(52/312)。低密度脂蛋白胆固醇高、术后LR11高、术后PTPRO高为AMI患者PCI术后ISR的独立危险因素(均P<0.05)。术后外周血LR11、PTPRO及二者联合预测AMI患者PCI术后ISR的曲线下面积为0.824、0.797、0.904,二者联合优于各自单独预测(均P<0.05)。结论 老年AMI患者PCI术后外周血LR11、PTPRO水平升高与PCI术后ISR有关,二者联合对PCI术后ISR有较高的预测效能。
Objective To investigate the changes of low-density lipoprotein receptor-related protein with 11 ligand-binding repeats (LR11) and protein tyrosine phosphatase receptor O (PTPRO) in peripheral blood before and after percutaneous coronary intervention (PCI) in elderly patients with acute myocardial infarction (AMI) and their relationship with in-stent restenosis (ISR). Methods A total of 312 elderly AMI patients who underwent PCI in Honghui Hospital Affiliated to Xi′an Jiaotong University from January 2021 to December 2023 were prospectively selected, and they were divided into ISR group and non-ISR group according to whether ISR occurred after 1 year. The levels of LR11 and PTPRO in peripheral blood were detected by enzyme-linked immunosorbent assay. Spearman correlation was used to analyze the correlation between the levels of peripheral blood LR11 and PTPRO and the rate of luminal diameter stenosis at 1 year after PCI in AMI patients, and the relationship between the levels of peripheral blood LR11 and PTPRO and ISR in AMI patients after PCI and its predictive efficacy were analyzed. Results The levels of LR11 and PTPRO in peripheral blood of AMI patients after PCI were higher than those before PCI [(195±48)mg/L vs (153±43)mg/L, (20±4)μg/L vs (16±4)μg/L](both P<0.05). The levels of LR11 and PTPRO in peripheral blood of AMI patients after PCI were positively correlated with the lumen diameter stenosis rate at 1 year after PCI (rs=0.617, 0.602, both P<0.001). After 1-year follow-up, the incidence of ISR after PCI was 16.7% (52/312) in 312 AMI patients. High low-density lipoprotein cholesterol, high postoperative LR11 and high postoperative PTPRO were independent risk factors for ISR in AMI patients after PCI (all P<0.05). The area under the curve of postoperative peripheral blood LR11, PTPRO and their combination to predict ISR in AMI patients after PCI was 0.824, 0.797 and 0.904, respectively, and the combination of the two was better than each single prediction (both P<0.05). Conclusion The increased levels of LR11 and PTPRO in peripheral blood of elderly AMI patients after PCI are related to ISR after PCI. The combination of LR11 and PTPRO has a high predictive effect on ISR after PCI.
